Rooibos + Perampanel
Perampanel brand name: Fycompa.
Perampanel falls into 4 drug categories that Rooibos can interact with; the most serious is rated moderate. Theoretically, taking rooibos with CYP2C19 substrates may increase the effects of CYP2C19 substrates.

These two interact in 4 distinct ways. Each one is explained separately below.
Why Are There 4 Interactions?
Rooibos interacts with whole categories of medication. Perampanel falls into 4 of those categories, so it can interact with Rooibos in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES
1 of 4Why this applies to Perampanel
Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Rooibos interacts with.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP2C19 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with rooibos and your seizure medicine. Your body uses a liver enzyme called CYP2C19 to help clear perampanel. In lab tests (just test tubes and animals, not real people), concentrated rooibos extract slowed that enzyme down. If that were to happen in your body, perampanel could build up a bit more than usual, which might mean stronger side effects like drowsiness, dizziness, trouble with balance, or mood changes such as irritability.
The good news: this is only a theoretical concern based on weak evidence, and a normal cup or two of rooibos tea is very unlikely to cause problems. Concentrated supplement extracts are a bigger question mark. If you plan to use those, just run it by your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 4Perampanel also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP1A2 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP1A2 to help clear perampanel (Fycompa) out of your system. In lab studies, rooibos extract appears to slow that enzyme down a bit. If that happened in real life, perampanel could build up slightly and hit harder, meaning more chance of drowsiness, dizziness, trouble with balance, or feeling irritable or foggy.
The honest part: this is only based on test-tube research, not real people, so it's more of a 'keep an eye out' than a red flag. A cup or two of rooibos tea is unlikely to cause trouble. Concentrated rooibos extracts are the bigger question mark. Since perampanel is a seizure medicine, it's worth a quick word with your pharmacist or doctor before adding supplement-strength rooibos.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
3 of 4Perampanel also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2C9 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Fycompa (perampanel): your body partly relies on a liver enzyme called CYP2C9 to break this seizure medicine down. Rooibos, in early lab testing, appears to slow that same enzyme a bit. In theory, that could let perampanel build up a little more than usual, which might nudge up its typical side effects like drowsiness, dizziness, trouble with balance, or mood and irritability changes.
I want to be honest about the strength here. This is based only on test-tube studies using concentrated rooibos extracts, not real people drinking the tea. A normal cup or two of rooibos is very unlikely to cause trouble. Concentrated extracts are the bigger question mark. Since perampanel needs steady levels to control seizures, just run it past your pharmacist or doctor before adding extracts.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
4 of 4Perampanel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your body uses an enzyme called CYP3A4 to break down perampanel (Fycompa). Lab studies hint that concentrated rooibos extract may slow that enzyme down. In theory, that could let perampanel build up a bit more than usual, which might mean stronger effects like drowsiness, dizziness, trouble with balance, or mood and irritability changes.
Now, the honest part: this is based only on test-tube research, not on real people. A regular cup or two of rooibos tea is very unlikely to cause trouble. The bigger question mark is with concentrated rooibos supplements or extracts. If you'd like to use those, just run it by your pharmacist or doctor first so they can keep an eye on how you're feeling.

From our pharmacist’s overview on the Perampanel page
“Perampanel (Fycompa) is a seizure medicine that already causes drowsiness and dizziness on its own, which is why I treat this list of 230 interacting supplements with real care. Most entries do one of two things: they add to its sedating effect, as alcohol and kava do, or they change how the liver handles it.”

About Rooibos
What is Rooibos?
- Caffeine-free beverage
- Antioxidant support
- Heart health
- Blood sugar support
- Digestive upset
- General wellness
Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.
About Perampanel
What Is Perampanel Used For?
Brand name: Fycompa
Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.
Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
