Rooibos and Testosterone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Testosterone

Testosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses an enzyme called CYP3A4 to help break down testosterone. In lab dish studies, a concentrated rooibos extract slowed that enzyme down. If that happened in people, testosterone could clear a bit slower and build up, which in theory might nudge up its effects and side effects (think acne, oily skin, mood changes, fluid retention, or changes in blood counts).

But keep this in perspective. This is only test-tube research, not something proven in real patients, and it involved strong rooibos extracts, not the cup of tea you'd sip. A normal mug of rooibos is very unlikely to matter here. If you're using concentrated rooibos supplements alongside your testosterone, just run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These are real questions from readers, answered by the pharmacists at HelloPharmacist — written by a person, not generated from a database. See Every Supplement That Interacts With Testosterone

This page covers Rooibos only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Testosterone page lists 170 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Beer (major), Grapefruit (major), Siberian Cocklebur (major), St. John's Wort (major) and Wine (major).

By severity: 5 major · 129 moderate · 36 minor.

From our pharmacist’s overview on the Testosterone page

“The supplement list for testosterone therapy (Androgel, Testim, Nebido, and others) deserves real attention, mainly because five of its 170 entries are rated major and four of those are everyday items: grapefruit, St. John's wort, and alcohol in the form of beer or wine.”

About Rooibos

What is Rooibos?

Rooibos
  • Caffeine-free beverage
  • Antioxidant support
  • Heart health
  • Blood sugar support
  • Digestive upset
  • General wellness

Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.

About Testosterone

What Is Testosterone Used For?

Brand names: Intrinsa, Testim, Androgel, Andropatch, Testogel, Nebido, Delatestryl, Natesto, Vogelxo

Testosterone replacement therapy is used in adult males whose bodies do not produce enough of their own testosterone. This includes primary hypogonadism — where the testes themselves fail due to causes such as Klinefelter's syndrome, injury, chemotherapy, or surgical removal — and hypogonadotropic hypogonadism, where the brain's signaling system (the hypothalamus or pituitary gland) fails to trigger testosterone production.

Testopel (subcutaneous pellets) may also be used to stimulate puberty in carefully selected males with clearly delayed puberty. The safety and efficacy of testosterone for age-related (late-onset) hypogonadism have not been established for most products, including AndroGel, Testim, Jatenzo, Kyzatrex, Natesto, Depo-Testosterone, Testosterone Cypionate, and Xyosted.
