Rooibos and Formoterol

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Rooibos interacts with whole categories of medication. Formoterol falls into 3 of those categories, so it can interact with Rooibos in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 3

Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP2C19 substrates may increase the effects of CYP2C19 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP2C19 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture with rooibos and your formoterol inhaler. The concern on file is that compounds in rooibos might slow down a liver enzyme called CYP2C19. In theory, slowing that enzyme could let a drug it clears build up a bit, which for formoterol might mean more of its side effects, things like a fast or pounding heartbeat, shaky hands, headache, or feeling jittery.

But I want to keep this in perspective. This warning comes only from lab and test-tube studies, not from people, and a regular cup of rooibos tea is very different from a concentrated extract. For most folks, sipping rooibos should be fine. If you use strong rooibos supplements or notice new jitteriness or a racing heart, check in with your pharmacist or doctor.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 3

Formoterol also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, taking rooibos with CYP2D6 substrates may increase the effects of CYP2D6 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2D6 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses an enzyme called CYP2D6 to help clear formoterol. In test-tube studies, rooibos extract slowed that enzyme down a bit. In theory, that could let a little more formoterol linger in your system, which might nudge up the usual beta-agonist effects like a faster or pounding heartbeat, shakiness or tremor, jitteriness, or a touch of headache.

Now the reassuring part. This is only lab-dish evidence, not something proven in people, and a cup of rooibos tea is far weaker than a concentrated extract. Most folks sipping it as a tea won't notice anything. Still, if you use a strong rooibos supplement or feel new heart racing or shakiness, check in with your pharmacist or doctor before changing anything.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

3 of 3

Formoterol also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, taking rooibos with CYP2C9 substrates may increase the effects of CYP2C9 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP2C9 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: rooibos tea and your formoterol inhaler are very unlikely to cause a real problem. The concern on file is purely theoretical. In a lab dish, a concentrated rooibos extract slowed down an enzyme called CYP2C9, and if that happened in the body, a drug cleared by that enzyme could build up a bit and have stronger effects.

For formoterol, this is reassuring. It's an inhaled medicine that mostly works right in your lungs and doesn't rely heavily on CYP2C9. If levels did creep up, the things to notice would be a faster or pounding heartbeat, shakiness, or feeling jittery. Drinking rooibos as a regular tea shouldn't matter. If you're using concentrated rooibos capsules, just mention it to your pharmacist or doctor.

From our pharmacist’s overview on the Formoterol page

“Formoterol (Foradil, Perforomist) is the long-acting half of many asthma and COPD inhalers, keeping airways open for hours, and its supplement list is calmer than its length suggests. None of the 103 ingredients on file is rated major.”

About Rooibos

What is Rooibos?

Rooibos
  • Caffeine-free beverage
  • Antioxidant support
  • Heart health
  • Blood sugar support
  • Digestive upset
  • General wellness

Rooibos is a caffeine-free herbal tea from South Africa that is rich in antioxidants and generally considered safe as a beverage. While lab and early studies suggest possible benefits for heart health, blood sugar, and inflammation, strong human evidence is lacking for most medical uses. Enjoy it as a tea, but talk to your pharmacist or doctor before using concentrated extracts for health reasons.
