Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C9 substrates.
Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Opsynvi combines two medicines, macitentan and tadalafil, and both are partly cleared by a liver enzyme called CYP2C9. The idea here is that bacopa might slow that enzyme down a little. If that happened, these drugs could build up in your body and their effects could be stronger than intended. For tadalafil, that might mean more headache, flushing, stuffy nose, or lightheadedness, and for macitentan, possibly more swelling or fatigue. Since both work on blood pressure, a bigger drop in blood pressure is the main thing to watch for.

That said, the evidence for this is weak, coming only from lab studies, and results even conflict on whether bacopa affects this enzyme at all. I wouldn't lose sleep over it, but do mention any bacopa supplement to your pharmacist or doctor first.

If it does, these two drugs could build up slightly, which in theory could nudge up their effects and side effects. With tadalafil that might mean more headache, flushing, stuffy nose, or lightheadedness, and both drugs together can lower blood pressure.

Here is the reassuring part: this is based only on lab studies, and the results actually conflict. Some suggest a modest effect, others suggest it likely doesn't matter in real life. Neither drug is a prodrug, so the direction points toward a bit more effect, not less. Still, since blood pressure is involved, it's worth mentioning bacopa to your pharmacist or doctor before combining them.

Bacopa is an Ayurvedic herb most often used for memory and thinking.
