Mometasone, formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the honest picture with Zenhale (mometasone and formoterol) and bacopa. The concern on file is that bacopa might slow down a liver enzyme called CYP2C19, which in theory could let certain medicines build up a bit. But this is based only on lab dish studies, not on real people, so we genuinely don't know if it matters in everyday use.

On top of that, your Zenhale is an inhaler. The medicine works mostly in your lungs, and very little reaches the bloodstream, so a liver enzyme effect is unlikely to cause much. If it did nudge levels up, you might notice more of formoterol's usual effects like a faster heartbeat, jitteriness, or shakiness. I wouldn't lose sleep over this, but it's a quick thing to mention to your pharmacist or doctor before starting bacopa.

If that happened with your inhaler, you might notice more of formoterol's typical effects, like a faster or pounding heartbeat, jitteriness, or a slight tremor in your hands.

That said, I wouldn't lose sleep over this one. The evidence is only from lab and animal studies, and even those results disagree about whether bacopa affects this enzyme in a meaningful way in people. Inhaled medicines also reach the bloodstream in fairly small amounts. Still, if you take bacopa regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

Here's the gist with your Zenhale inhaler (mometasone plus formoterol). Both of these medicines are partly cleared by a liver enzyme called CYP3A4. In test-tube studies, bacopa might slow that enzyme down a little. If it does, slightly more of the drug could hang around, which in theory could mean a bit more steroid effect or more of the formoterol-type side effects like a faster heartbeat, jitteriness, shakiness, or trouble sleeping.

I'll be honest though, this is very weak, theory-only evidence, and lab results even disagree with each other. Inhaled doses also mostly act in the lungs, so a real-world problem is unlikely. Neither of these is a prodrug, so the worry is buildup, not lost benefit. Still, mention bacopa to your pharmacist or doctor so they can keep an eye on things.

Bacopa is an Ayurvedic herb most often used for memory and thinking.
