This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). The body uses several liver enzymes to clear these, and one of them is CYP1A2. The thought here is that bacopa might slow CYP1A2 a bit, which could let these medicines build up slightly. If that happened, you might notice more of their usual side effects, like drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat.

I want to be honest about how shaky this is. The concern comes only from lab and test-tube studies, and even those disagree, so we don't know it matters in real people. This combo pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic), and both carry strong anticholinergic activity, meaning they work partly by blocking acetylcholine in the body. Bacopa appears to nudge things the opposite way, raising acetylcholine levels. So in theory the two could partly cancel each other out. The medication might feel a little less effective, and bacopa's hoped-for memory benefit could be blunted too.

I want to be honest with you though: this is a theoretical concern based on how these things work in the lab, not from real patient reports, so I wouldn't lose sleep over it. Bacopa is an Ayurvedic herb most often used for memory and thinking.
