Amitriptyline, Perphenazine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Black Hoof Mushroom interacts with.

Theoretically, black hoof mushroom might increase the levels and clinical effects of CYP1A2 substrates.
In vitro, polysaccharides from black hoof mushroom grown in culture inhibit CYP1A2. This effect has not been reported in humans.

Triavil combines amitriptyline, an older antidepressant, with perphenazine, an antipsychotic. Your liver uses an enzyme called CYP1A2 (among others) to help break amitriptyline down. In lab dish studies, compounds from black hoof mushroom slowed that enzyme. If the same thing happened in your body, this medicine could clear a little more slowly and build up, which might mean stronger effects like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

Here's the honest part: this has only been seen in test tubes, never in people. So it's a theoretical concern, not a proven one. Still, since amitriptyline can be sensitive to changes, please run this combo by your pharmacist or doctor before adding the mushroom.
