This combo pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Your liver uses an enzyme called CYP3A4 to help clear these medicines. In lab dish studies, American elder appears to slow that enzyme down a bit. The worry, in theory, is that the drug could linger longer and build up, nudging up its usual side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat.

Here's the honest part: this is based only on test-tube research, not on real people, so it's a soft, theoretical concern. Small culinary amounts of properly cooked elderberry are unlikely to cause trouble, but concentrated supplements are less certain. Before adding elderberry, just run it by your pharmacist or doctor.
