These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Bacopa interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 2 of those categories, so it can interact with Bacopa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Aspirin, Caffeine, Codeine Phosphate
Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product has three pieces, and the part that matters here is the caffeine. Caffeine is cleared from your body mostly by a liver enzyme called CYP1A2. In test-tube studies, bacopa has shown a mild ability to slow that enzyme down. If that held true in people, caffeine could linger a bit longer and you might feel jittery, have a racing heart, trouble sleeping, or an upset stomach.
The honest takeaway: this is theoretical and comes only from lab studies, and even that evidence is mixed, so for most people it is unlikely to be a real problem. The aspirin and codeine parts are not meaningfully affected. Still, if you notice extra restlessness or your heart pounding, mention it to your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill (aspirin, caffeine, and codeine). In lab dishes, bacopa might slow down a liver enzyme called CYP3A4, which helps clear certain drugs. If that happened in your body, a bit more of a drug could linger and effects could feel stronger. For the codeine portion, that could mean more drowsiness, constipation, or an upset stomach, and the caffeine piece can add jitteriness or trouble sleeping.
That said, this is only a theoretical concern from test-tube studies, and the research is honestly mixed. Most people won't notice anything. Codeine actually relies on a different enzyme (CYP2D6) to work, so the link here is shaky. Still, before pairing them regularly, run it by your pharmacist or doctor just to be safe.

From our pharmacist’s overview on the Aspirin, Caffeine, Codeine Phosphate page
“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”
