This combination pill contains codeine, an opioid that can make you drowsy and slow your breathing. When you add CBD, the main concern is that both can have a calming, sedating effect, and those effects may stack on top of each other. That could leave you feeling more sleepy, foggy, lightheaded, or unsteady than usual, especially with higher CBD doses.

Keep in mind this is mostly a theoretical worry based on how the ingredients work, not strong human proof, so I don't want to alarm you. Still, since codeine already carries real risks, it's worth a quick chat with your pharmacist or doctor before pairing the two, and avoid driving until you know how you feel.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the wrinkle with this combo product: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. CBD can mildly block that same enzyme in lab studies, so in theory it could leave you with less active morphine, meaning weaker pain relief rather than more side effects from the codeine.

I want to keep this in perspective, though. This is a theoretical concern based mostly on test-tube data, and a real human study using a similar CYP2D6 drug showed no meaningful blocking. The aspirin and caffeine pieces aren't really involved here. If you use CBD regularly and notice your pain medicine isn't working as well, mention it to your pharmacist or doctor.

In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the piece that matters with this combo: the caffeine in this product is cleared by an enzyme called CYP1A2, and CBD can slow that enzyme down. In good-quality studies, people who took CBD ended up with more caffeine hanging around in their blood. So pairing CBD with this medicine could leave you feeling extra wired, jittery, anxious, or with a racing heart, trouble sleeping, or an upset stomach.

The aspirin and codeine parts aren't the main concern here, and food or drink amounts of CBD are unlikely to do much. It's the larger, concentrated CBD supplement doses that tend to matter. If you regularly use CBD, it's worth a quick chat with your pharmacist or doctor before combining the two.

This combination pill packs three ingredients, and CBD touches them in different ways. The piece that matters most here is the codeine. Codeine is a prodrug, meaning your body has to convert it into morphine before it relieves pain, and that activation depends mainly on an enzyme called CYP2D6, not the CYP3A4 pathway that CBD blocks. CYP3A4 actually shunts codeine toward an inactive byproduct, so slowing it could leave a little more codeine to become morphine. In plain terms, you might notice stronger opioid effects like extra drowsiness, constipation, or slowed breathing.

The aspirin and caffeine parts are not strongly affected. Effects here are modest and vary by person, so before mixing CBD with this medicine, check in with your pharmacist or doctor.

Caffeine is a substrate of CYP1A2, and cannabidiol has been shown to inhibit CYP1A2 metabolism. A pharmacokinetic study in healthy adults shows that taking oral cannabidiol, starting at 250 mg once daily and titrating to 750 twice daily over a total of 24 days, increases the peak serum level of caffeine by 15% and the overall exposure to caffeine by 95% after a single dose of caffeine 200 mg taken on day 23. Other clinical research also shows that cannabidiol modestly increases the area under the curve of caffeine but does not increase peak serum levels of caffeine.

Here's the piece to focus on with this combo pill: it contains caffeine, and your body clears caffeine using a liver enzyme called CYP1A2. CBD can slow that enzyme down. In a good-quality study, higher daily CBD doses nearly doubled the body's overall exposure to a single dose of caffeine. In plain terms, the caffeine in this product may stick around longer and hit a little harder.

What that might feel like is more of caffeine's usual effects: jitteriness, a racing or pounding heart, trouble sleeping, or feeling on edge. The aspirin and codeine parts aren't really the issue here. This is usually manageable, but if you use CBD regularly, mention it to your pharmacist or doctor so they can help you watch your caffeine intake.
