This combination pill contains codeine, an opioid that can make you feel drowsy and relaxed. The concern here is mostly about that codeine piece. In animal studies, calamus seems to act as a mild nervous-system depressant by raising a calming brain chemical called GABA. Stack that on top of codeine and, in theory, the two could add together and leave you more sedated than expected.

What might that look like? Extra sleepiness, foggy thinking, slowed reactions, or in a worse case slower breathing. To be fair, this is a theoretical worry based on lab and animal work, not solid human proof. Calamus also has its own safety baggage (it's FDA-banned in foods), so it's worth a quick chat with your pharmacist or doctor before mixing them.

Here's the wrinkle with this combo product: the codeine part is a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Lab studies suggest calamus may block that enzyme. So instead of building up, codeine could end up doing less for you, meaning weaker pain relief from this medication.

I want to be honest that this is only theoretical, based on test-tube research, not real people, so it may not amount to much in practice. The aspirin and caffeine pieces aren't really affected. Honestly, though, calamus carries its own safety baggage (it's banned from foods in the U.S.), so I'd skip it. Please check with your pharmacist or doctor before mixing the two.

This combination pill mixes aspirin, caffeine, and codeine. The piece that matters here is the codeine. Codeine is a prodrug, meaning your body has to convert it into its active painkiller (morphine). A different enzyme, CYP2D6, does most of that conversion, while CYP3A4 mostly shunts codeine down a side path. The lab data suggest calamus may block CYP3A4. In theory, that could leave a bit more codeine to take the active route, which might mean a touch more drowsiness, constipation, nausea, or slowed breathing.

That said, this is only test-tube research with no human proof, so I'd call it shaky. Calamus also carries its own safety concerns and is best avoided. Please check with your pharmacist or doctor before pairing the two.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
