Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist with Cabenuva (cabotegravir plus rilpivirine) and bacopa. Rilpivirine is partly broken down by a liver enzyme called CYP3A4. In test-tube studies, bacopa has shown a possible ability to slow that enzyme down. If that happened in the body, rilpivirine could build up a bit, which might mean more side effects like headache, trouble sleeping, dizziness, mood changes, or nausea.

I want to be honest, though: this is a theoretical concern based only on lab data, and the studies even disagree with each other. There's no good evidence it causes real problems in people. Since Cabenuva is your HIV treatment, I wouldn't want anything throwing it off, so just run bacopa by your pharmacist or doctor first before adding it.

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
