Aspirin, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with anticoagulant/antiplatelet drugs might increase the risk of bleeding. In vitro research shows that certain flavonoid constituents from breadfruit root extract can inhibit thromboxane formation and reduce platelet aggregation.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

This combination pill contains aspirin, which already thins your blood a bit by blocking platelets from clumping together. The concern here is that breadfruit, especially concentrated leaf or root extracts, may also nudge platelets in the same direction. Stacking two blood-thinning effects could, in theory, make bleeding or bruising a little easier, things like nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest about the strength of this: it comes only from lab (test tube) studies, not from people, so it's a theoretical caution rather than something proven. Eating breadfruit as a normal food is very unlikely to matter. If you're using breadfruit supplements regularly, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the practical picture with this combo pill. The codeine part is actually a prodrug, meaning your body has to convert it into morphine to get pain relief, and that activation mostly runs through a different enzyme (CYP2D6), not the CYP3A4 pathway breadfruit may touch. So this particular interaction isn't a major concern for how well the codeine works.

The flagged worry is that breadfruit might mildly slow CYP3A4, which could let a bit more codeine linger. In theory that could nudge up drowsiness, constipation, or nausea. But honestly, this is only based on lab and animal studies, so the real-world effect is uncertain and likely small. Eating breadfruit as food shouldn't matter. If you're using concentrated leaf teas or extracts, just run it past your pharmacist first.
