Acetaminophen, Chlorpheniramine, Phenylephrine brand names: Protid, Histex SR, Alka-Seltzer PLUS.
Acetaminophen, Chlorpheniramine, Phenylephrine falls into 3 drug categories that Bacopa can interact with; the most serious is rated moderate. Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Bacopa interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Phenylephrine falls into 3 of those categories, so it can interact with Bacopa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Chlorpheniramine, Phenylephrine
Acetaminophen, Chlorpheniramine, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this cold-and-allergy combo. In lab dishes, bacopa may mildly slow down a liver enzyme called CYP1A2. In theory, that could let a medicine cleared by that enzyme hang around a little longer. For this product, that might mean a touch more of the usual side effects, like drowsiness, dry mouth, and a foggy feeling from the chlorpheniramine, or jitteriness and a faster heartbeat from the phenylephrine.
That said, the evidence here is weak (test-tube only) and even those studies disagree, so a real-world problem is far from certain. None of these ingredients lean heavily on CYP1A2 either. Still, if you notice extra sleepiness or your heart racing, check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Chlorpheniramine, Phenylephrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold product (acetaminophen for pain and fever, chlorpheniramine for allergy symptoms, and phenylephrine as a decongestant) is partly handled by a liver enzyme called CYP3A4. In lab dishes, bacopa has shown some ability to slow that enzyme down, which in theory could let a bit more of these ingredients linger in your body. If that happened, you might notice a little more drowsiness or dry mouth from the chlorpheniramine, or more jitteriness from the phenylephrine.
That said, the evidence here is weak. It comes only from test-tube and animal studies, and even those results disagree. For most people taking the occasional dose, this is unlikely to cause real trouble. Still, if you use bacopa regularly, it's worth a quick word with your pharmacist or doctor.

ANTICHOLINERGIC DRUGS
3 of 3Acetaminophen, Chlorpheniramine, Phenylephrine also falls under ANTICHOLINERGIC DRUGS.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece to focus on with this combination product: the chlorpheniramine. That's an older antihistamine that works partly by blocking acetylcholine, one of the brain's chemical messengers. Bacopa appears to do the opposite in lab studies, nudging acetylcholine levels up. So in theory, the two could partly cancel each other out. Bacopa might make the chlorpheniramine work a little less well, and the chlorpheniramine could blunt any memory benefit you're hoping for from bacopa.
I want to be honest though: this is based on how these things work on paper, not on real-world studies in people, so it's a fairly soft concern. The acetaminophen and phenylephrine parts aren't really involved here. Still, it's worth a quick mention to your pharmacist or doctor before pairing them regularly.

- Memory and cognitive support
- Anxiety and stress
- ADHD symptoms
- Age-related memory decline
- General brain health
Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.
