Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the basic idea with bacopa and Selpercatinib (Retevmo). Your body relies on a liver enzyme called CYP3A4 to break down and clear this cancer medicine. In lab dish studies, bacopa has shown a mild ability to slow that same enzyme down. If that happened in a real person, more selpercatinib could linger in the bloodstream, which could mean more side effects like high blood pressure, swelling, nausea, fatigue, diarrhea, or changes in liver or heart (QT) readings.

That said, this is theoretical. It comes only from test-tube research, and even that research disagrees on whether the effect matters at all. With a serious cancer drug, though, I wouldn't guess. Please run bacopa past your oncology team or pharmacist before adding it.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
