First, the most important thing: European mandrake is genuinely poisonous, and I would not recommend taking it at all, especially alongside a serious cancer medicine like selpercatinib (Retevmo). Selpercatinib works as-is, so it is not a prodrug that needs activating, this is purely about how much of it your gut absorbs.

The concern here is theoretical and based on pharmacology, not solid studies. Mandrake's tropane alkaloids can slow down how fast your gut empties. When things move slower, your body may absorb a bit more of an oral drug, which could in theory raise selpercatinib levels and make side effects like high blood pressure, nausea, swelling, dry mouth, or liver strain more likely. Please don't change anything on your own, just loop in your oncology team or pharmacist.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
