Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Here's the concern with pairing Puravive and Selpercatinib (Retevmo). Selpercatinib can lengthen something called the QT interval, which is part of your heart's electrical rhythm. Puravive contains Panax ginseng, and some short-term studies suggest ginseng may stretch that interval a little too. Stacked together, the worry is an additive effect that, in rare cases, could trigger an abnormal heart rhythm.

I'll be honest, the evidence here is mixed and not strong. Some studies showed a small change, others showed none with longer use. So this is more of a caution than a sure thing. Still, because Selpercatinib treats serious cancers and heart rhythm matters, please check with your oncologist or pharmacist before adding Puravive rather than starting it on your own.

Selpercatinib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Retevmo (selpercatinib). This cancer medicine is processed by a liver enzyme called CYP3A4. Puravive contains Panax ginseng, which has shown mixed effects on that same enzyme. In some studies it appeared to slow CYP3A4 down (which could let selpercatinib build up, raising the odds of side effects like high blood pressure, liver enzyme changes, swelling, or QT-related heart rhythm issues). In others it sped the enzyme up (which could lower selpercatinib levels and make it work less well against your cancer).

Honestly, the evidence here is weak and comes from other drugs, not selpercatinib itself, so I wouldn't panic. But because this is serious cancer therapy, please run Puravive past your oncologist or pharmacist before starting it.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
