Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

This combo product pairs diltiazem (a calcium channel blocker) with enalapril (an ACE inhibitor) to lower blood pressure. The diltiazem part is processed by a liver enzyme called CYP3A4. In test-tube studies, bacopa may slightly slow that enzyme down, which could in theory let diltiazem build up a bit higher than usual.

If that happened, you might notice more of diltiazem's typical effects, like a slower heart rate, lower blood pressure, dizziness, or swollen ankles. But honestly, the evidence here is weak and conflicting (just lab data), so a real-world problem is far from certain. I wouldn't lose sleep over it, but it's worth mentioning to your pharmacist or doctor, especially if your blood pressure or pulse tends to run low.

Bacopa is an Ayurvedic herb most often used for memory and thinking.
