Why this applies to Roflumilast
Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Danshen and your COPD medicine Roflumilast (Daliresp or Zoryve). Your liver and gut use an enzyme called CYP3A4 to help process roflumilast. The early human research we have suggests that taking Danshen for a week or two actually speeds this enzyme up, which would clear roflumilast out of your body faster. In plain terms, that could make it work a little less well at calming the swelling in your lungs, so your COPD might not be as well controlled.
To make things trickier, a single dose of Danshen seemed to do the opposite and raise drug levels, which could mean more side effects like nausea, diarrhea, weight loss, or trouble sleeping. The evidence here is limited, so please don't change anything on your own. Just check with your pharmacist or doctor before combining the two.

What Is Roflumilast Used For?

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.
Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
