Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for your COPD medicine. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4, and that's the same enzyme that helps activate roflumilast. Roflumilast is actually turned into an active form (roflumilast N-oxide) that does much of the work to calm swelling in your airways. If the enzyme runs faster, your body may clear the drug a bit quicker, which could mean slightly less protection against COPD flare-ups.

The good news: this is a theoretical concern based on how the drugs work, and modafinil is only a weak booster of that enzyme, so a big problem is unlikely. Still, since adrafinil is unregulated and hard on the liver, please check with your doctor or pharmacist before combining them.

About Roflumilast

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
