Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture: your body uses a liver enzyme called CYP3A4 to help break down roflumilast (Daliresp). Grape seed extract might affect that enzyme, but the research pulls in two opposite directions. Lab dish studies hint it could slow the enzyme down, while animal studies suggest it might speed it up. So we genuinely can't say which way it would push roflumilast levels.

If levels crept up, you'd watch for roflumilast's usual side effects (nausea, diarrhea, headache, trouble sleeping, weight loss). If levels dropped, your COPD might not be as well controlled. The good news: this has never actually been reported in people, so it's theoretical right now. Eating grapes is fine. Before starting a concentrated grape seed supplement, just run it by your pharmacist or doctor.

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
