Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to help clear roflumilast (Daliresp, Zoryve). In lab dish studies, uva ursi appears to slow that same enzyme down. If that were to happen in real life, roflumilast could build up a bit and its usual side effects might feel stronger, things like nausea, diarrhea, headache, trouble sleeping, weight loss, or low mood.

I want to be honest, though: this is only a theoretical, test-tube finding, and it has not been seen in actual people. So I wouldn't lose sleep over it. Still, uva ursi is only meant for short-term use and can stress the liver. Before pairing these, it's worth a quick chat with your pharmacist or doctor.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Roflumilast

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
