Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist for Roflumilast (Daliresp, Zoryve): your liver uses an enzyme called CYP3A4 to help process this medicine. In test-tube studies, boswellia appears to block that enzyme, which could in theory slow how fast Roflumilast is cleared. If the drug lingers a bit longer, you might notice more of its usual side effects, like nausea, diarrhea, headache, trouble sleeping, or some weight loss.

I want to be honest about how shaky this is: it's only a theory based on lab research, not real patient cases, so for most people it likely won't cause noticeable problems. Still, since Roflumilast can already be hard on the stomach, it's worth a quick mention to your pharmacist or doctor before pairing the two, especially with concentrated boswellia supplements.

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
