Why this applies to Roflumilast

Roflumilast falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: black psyllium is mostly a fiber supplement, and that thick gel it forms in your gut can sometimes grab onto medicine you swallow and change how much gets absorbed. With roflumilast (Daliresp, Zoryve), this is a minor, theoretical concern. The fiber could in theory either slow down or slightly reduce how much of your daily pill you take in, which might mean a touch less benefit for your COPD.

The easy fix is timing. Take your roflumilast first, then wait about 30 to 60 minutes before your psyllium. That keeps the fiber from interfering. This is low-level evidence, so don't panic, but if you notice your breathing isn't as well controlled, mention it to your pharmacist or doctor.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. Talk with your pharmacist or doctor before starting, especially if you have swallowing problems or bowel disorders.

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
