Why this applies to Roflumilast

Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for roflumilast (Daliresp, Zoryve): your liver uses an enzyme called CYP3A4 to help break it down. Breadfruit contains compounds that, in lab and animal studies, may gently slow that same enzyme. If that slowdown happened in people, roflumilast could build up a bit and you might notice more of its usual side effects, like nausea, diarrhea, headache, trouble sleeping, or some weight loss.

But I want to keep this in perspective. This is only theoretical, based on test-tube and animal work, not real human results. Eating breadfruit as a food is very unlikely to cause a problem. Concentrated leaf teas or extracts are the bigger unknown. If you're using those, just run it by your pharmacist or doctor first.

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Roflumilast tablets (Daliresp and generic roflumilast) are used to reduce the risk of COPD flare-ups (exacerbations). They are for people with severe COPD associated with chronic bronchitis who have a history of flare-ups.

Zoryve is the skin form. Zoryve cream treats plaque psoriasis and mild to moderate atopic dermatitis. Zoryve foam treats seborrheic dermatitis and plaque psoriasis of the scalp and body. Each product has its own approved ages.
