Tangerine has a minor interaction with Alpelisib. In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4).

Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: your body clears alpelisib (Piqray) partly through a liver enzyme called CYP3A4. A natural compound in tangerine, called tangeretin, was shown in lab dishes to slightly speed up that enzyme. In theory, a faster enzyme could clear alpelisib a bit quicker and make it a touch less effective against your cancer.
The good news is this is very weak. When people actually drank tangerine juice in studies, it did not meaningfully change how a similar CYP3A4 drug was processed. So enjoying tangerines as food is very unlikely to throw off your alpelisib. Still, since this drug is treating breast cancer, it's worth a quick mention to your oncology team or pharmacist, especially if you're using concentrated tangerine or citrus peel supplements.

What Is Alpelisib Used For?
Brand name: Piqray
Piqray is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and PIK3CA-mutated. It is for use after the cancer has progressed on or after an endocrine-based (hormone-blocking) treatment. An FDA-approved test must detect the mutation.
Vijoice is used for adults and children 2 years and older with severe PROS who need systemic (whole-body) therapy. This use has accelerated approval, which is based on response rate and duration of response. Continued approval may depend on confirmatory trials.
