Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the situation with tangerine and your busulfan. A compound in tangerine called tangeretin has been shown in lab dishes to slightly speed up CYP3A4, one of the enzymes that helps clear busulfan from your body. In theory, faster clearance could lower busulfan levels a bit, which might mean slightly less of the effect you want against the leukemia cells.
The good news is this is mostly lab and animal data. When people actually drank tangerine juice, it didn't meaningfully change drug metabolism in studies. So enjoying tangerines as food is very unlikely to cause a real problem. That said, busulfan is a powerful cancer drug with careful dosing, so check with your oncology team or pharmacist before adding concentrated tangerine extracts or large amounts.

What Is Busulfan Used For?

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.
Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
