Why this applies to Fostamatinib

Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist for your platelet medicine, Tavalisse. Your liver turns fostamatinib into its active form (R406), and that active piece is then broken down by an enzyme called CYP3A4. The thought is that something in tangerine, a flavonoid called tangeretin, might gently speed up that enzyme. In theory, a faster enzyme could clear the drug a bit quicker and make it work less well, which for ITP could mean less help keeping your platelets up.

The good news: this is mostly lab-dish science. When people actually drank tangerine juice, it didn't meaningfully change how the body handled a similar CYP3A4 drug. So enjoying tangerines as food is very unlikely to be a problem. If you're using concentrated peel extracts or supplements, just mention it to your pharmacist or doctor.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
