Lomitapide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist for lomitapide (Juxtapid). Your body relies on an enzyme called CYP3A4 to clear this drug, and lomitapide is very sensitive to anything that touches that pathway. In the lab, a compound in tangerine (tangeretin) sped up CYP3A4 a bit, which could in theory lower lomitapide levels and make it work less well at controlling your LDL cholesterol.
The reassuring part: in actual people, tangerine juice did not change how a CYP3A4 drug was processed. So this concern is mostly theoretical, and eating tangerines as food is very unlikely to matter. Still, because lomitapide has a narrow safety margin and strict dosing rules, it's worth a quick check with your pharmacist or doctor before adding any concentrated citrus supplements.

What Is Lomitapide Used For?
Brand name: Juxtapid
Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.
Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
