Why this applies to Verapamil

Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Turpentine Oil interacts with.

There is some concern that turpentine oil can inhibit CYP1A2 enzyme activity by up to 90% via induction of inflammation and subsequent downregulation of CYP activity. However, animal research suggests that turpentine oil does not affect the metabolism of phenacetin, a CYP1A2 substrate, despite inducing inflammation.
Some medications metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, tizanidine (Zanaflex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the quick version on combining turpentine oil with your verapamil. The theory is that turpentine oil might slow down a liver enzyme called CYP1A2, which would let drugs cleared by that pathway build up a bit. If that happened with verapamil, you could in theory see a little more of its usual effects, like lower blood pressure, a slower heart rate, lightheadedness, swollen ankles, or constipation.

That said, I don't want to worry you. This concern comes only from lab and animal data, and one animal study showed no real change in how a CYP1A2 drug was processed. Verapamil is actually handled mostly by a different enzyme (CYP3A4), so this particular interaction is rated minor and unlikely. Just check with your pharmacist or doctor before regular use.

Turpentine oil is a strong-smelling pine resin product used mostly in tiny amounts in topical rubs for muscle aches and congestion. It can be dangerous and even deadly if swallowed, and reliable evidence for health benefits is very limited. Never drink turpentine oil, and only use diluted, purified products on the skin with care.

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
