Why this applies to Maprotiline

Maprotiline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Here's the good news on this pairing. On paper, valerian was flagged because maprotiline is partly handled by a liver enzyme called CYP2D6, and some test-tube studies hinted valerian might block it. But when researchers actually tested this in people, valerian did not meaningfully change how that enzyme works. So a real, clinically important boost in maprotiline blood levels from valerian is unlikely.

The bigger thing I'd watch is overlap in effects. Both maprotiline and valerian can make you drowsy or mellow, so taken together you might feel extra sleepy, foggy, or dizzy. That's not dangerous for most people, but please check with your pharmacist or doctor before adding valerian, and don't change your maprotiline on your own.

  • Maprotiline is used to treat depression, bipolar disorder (manic depressive disorder; a disease that causes episodes of depression, episodes of mania, and other abnormal moods), and anxiety. Maprotiline is in a class of medications called tetracyclic antidepressants. It works by increasing the amounts of certain natural substances in the brain that are needed to maintain mental balance.
