Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist with this headache combo (Tecnal, Esgic Plus and similar). One ingredient, the butalbital piece, is partly broken down by a liver enzyme called CYP3A4. In lab-based thinking, high-dose vitamin E might gently nudge that enzyme to work a little faster. If that happened, your body could clear the butalbital a bit quicker, which in theory might make the medication feel slightly less effective for relieving your tension headache.

I want to be honest though: this is theoretical, based on how these compounds behave in the lab, not on real patient reports. The everyday vitamin E in food or a normal multivitamin is very unlikely to matter. If you take a high-dose vitamin E supplement, just mention it to your pharmacist or doctor so they can keep an eye on things.

What Is Acetaminophen, Butalbital, and Caffeine Used For?

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).

All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
