Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Piqray (alpelisib). Your body clears this cancer medicine partly through a liver enzyme called CYP3A4. In lab-based theory, high-dose vitamin E may gently rev up that enzyme, which could cause your body to break down alpelisib a little faster. The worry there isn't extra side effects, it's the opposite: lower drug levels might make the treatment work less well against your breast cancer or PROS condition.
I want to be honest about how soft this is. It's theoretical, based on how vitamin E behaves in cells, not on real patient cases. Food-level vitamin E almost certainly won't matter; concentrated high-dose supplements are the real question. Since alpelisib is a serious cancer therapy, please run any vitamin E supplement by your oncologist or pharmacist first.

What Is Alpelisib Used For?
Brand name: Piqray
Piqray is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and PIK3CA-mutated. It is for use after the cancer has progressed on or after an endocrine-based (hormone-blocking) treatment. An FDA-approved test must detect the mutation.
Vijoice is used for adults and children 2 years and older with severe PROS who need systemic (whole-body) therapy. This use has accelerated approval, which is based on response rate and duration of response. Continued approval may depend on confirmatory trials.
