Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist for your HIV medication, Doravirine (Pifeltro). Your liver clears this drug using an enzyme called CYP3A4. In theory, high-dose vitamin E may nudge that enzyme to work a little faster. If that happens, your body could break down doravirine more quickly, which might lower its levels in your blood. Since doravirine isn't a prodrug, lower levels could mean it works less well at keeping HIV under control.

Now, please don't panic. This is a theoretical concern based on lab pharmacology, not real-world reports, and a normal diet's worth of vitamin E almost certainly won't matter. But because keeping HIV suppressed is so important, talk with your pharmacist or doctor before taking high-dose vitamin E supplements.

About Doravirine

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used to treat HIV-1 infection in adults and in children and adolescents who weigh at least 35 kg (about 77 lbs). It is always used in combination with other antiretroviral medicines — never on its own.

Pifeltro is approved for two groups: people who have never taken HIV treatment before, and people who are already virologically suppressed (meaning HIV is undetectable in their blood) on a stable regimen and need to switch, provided they have no history of treatment failure or known resistance to doravirine.
