Vitamin E and Lomustine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ALKYLATING AGENTS

Why this applies to Lomustine

Lomustine falls under ALKYLATING AGENTS — the group Vitamin E interacts with.

Theoretically, antioxidant effects of vitamin E might reduce the effectiveness of alkylating agents.
There's concern that antioxidants could reduce the activity of chemotherapy drugs which generate free radicals, such as cyclophosphamide, chlorambucil, carmustine, busulfan, and thiotepa. However, some researchers theorize that antioxidants might make chemotherapy more effective by reducing oxidative stress that might interfere with apoptosis (cell death) of cancer cells. More evidence is needed to determine what effect, if any, antioxidants such as vitamin E have on chemotherapy. Advise patients to consult their oncologist before using vitamin E supplements, especially in high doses.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thinking behind this one. Lomustine is a chemo drug that partly works by creating reactive, damaging molecules (free radicals) that harm cancer cells. Since vitamin E is an antioxidant, the worry is that taking it, especially in high doses, could mop up some of those free radicals and theoretically make Lomustine less effective against the tumor. That's the concern with brain tumors and Hodgkin's lymphoma you'd be treating.

I want to be honest: this is a theoretical idea based on how these drugs work, not proven in people. Some researchers even think antioxidants might help. Because the stakes with cancer treatment are high, please don't start or stop vitamin E on your own. Talk with your oncologist first, especially before any high-dose supplement.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Lomustine page

“One capsule of lomustine (Ceenu) often carries a whole chemotherapy cycle, so supplements that keep it from being absorbed, or that blunt its cancer-fighting action, deserve more respect here than with most drugs. The lone major-rated interaction, activated charcoal, can bind lomustine in the gut and sharply cut how much reaches the body.”
