Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist with voxelotor (Oxbryta) and vitamin E. Your body clears voxelotor mainly through a liver enzyme called CYP3A4. In the lab, high-dose vitamin E seems to nudge that enzyme to work faster. If that happens, your body could break down voxelotor a little quicker, which might lower its levels and make it work less well at protecting your red blood cells in sickle cell disease.

Now, I want to keep this in perspective. This is a theoretical concern based on how things behave in test tubes, not proven in real patients. Normal food amounts of vitamin E are very unlikely to matter. It's mainly the high-dose supplements worth a second look. Before adding one, check with your pharmacist or doctor so nothing weakens your treatment.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
