Aspirin, Butalbital falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits platelet aggregation. Theoretically, white pepper might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Some anticoagulant/antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Axotal combines aspirin with butalbital, and the part that matters here is the aspirin. Aspirin already works as a blood thinner by keeping your platelets from clumping together. The thinking is that piperine, the spicy compound in white pepper, may also nudge platelets in that same direction. Stack the two and you could, in theory, see a bit more bleeding risk, things like easier bruising, nosebleeds, or stomach upset from the aspirin.

Here is the reassuring part: this concern comes only from lab and animal studies, not from people, so the evidence is weak. The pinch of white pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are a different story, so check with your pharmacist or doctor before using those.
