Avanafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the short version: your body relies on an enzyme called CYP3A4 to break down avanafil (Stendra) and clear it out. Piperine, the spicy compound in white pepper, can mildly block that same enzyme in lab studies. If it slows down the breakdown, more avanafil could linger in your system, which may bump up its usual effects and side effects, things like headache, facial flushing, a stuffy nose, dizziness, or a drop in blood pressure.

The good news is this is theoretical and based on test-tube work, not real people. Normal kitchen amounts of white pepper on your food are very unlikely to matter. Concentrated piperine supplements are the bigger question mark, so check with your pharmacist or doctor before combining those.
