Hawthorn and Avanafil

Here is the interaction we found between these two, with the evidence behind it.

Major

PHOSPHODIESTERASE-5 INHIBITORS

Why this applies to Avanafil

Avanafil falls under PHOSPHODIESTERASE-5 INHIBITORS — the group Hawthorn interacts with.

Theoretically, concomitant use might result in additive vasodilation and hypotension.
Hawthorn might inhibit PDE-5 and cause vasodilation.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Stendra (avanafil) and hawthorn. Avanafil works by relaxing blood vessels so more blood flows to help with an erection. Hawthorn may do something similar, possibly widening blood vessels on its own. Put the two together and you could, in theory, get a little too much vasodilation. That can show up as low blood pressure, dizziness, lightheadedness, headache, or feeling faint when you stand up.

I want to be honest about the evidence here: this concern is based on how these things work in the lab, not on solid human studies, so it's more theoretical than proven. Still, the potential drop in blood pressure is worth respecting. Before combining them, please run it by your pharmacist or doctor.

From our pharmacist’s overview on the Avanafil page

“Avanafil (Stendra) treats erectile dysfunction by widening blood vessels, and I read this list of 144 supplements as one that warrants real care rather than panic. Most of the risk runs through a single liver enzyme, CYP3A4, which clears avanafil and which 121 of these ingredients can affect.”

About Hawthorn

What is Hawthorn?

Hawthorn
  • Heart failure symptoms
  • High blood pressure
  • Chest pain (angina)
  • Irregular heartbeat
  • Digestive upset
  • Anxiety

Hawthorn is a plant traditionally used for heart-related complaints, and some studies suggest it may modestly help symptoms of mild heart failure when added to standard care. However, the evidence is mixed and it should never replace prescribed heart medicines. Always talk to your pharmacist or doctor before using hawthorn, especially if you have a heart condition.
