In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the wrinkle with Vituz (chlorpheniramine plus hydrocodone): the hydrocodone part is partly a prodrug. Your body uses an enzyme called CYP2D6 to turn some of it into hydromorphone, which adds to the pain relief. Lab studies hint that compounds in white pepper may gently block that same enzyme. In theory, that could mean a little less of that conversion, so you might feel slightly less pain relief from the hydrocodone.

Honestly though, this is theoretical and based only on test-tube work, not real patients. The amount of white pepper you'd sprinkle on food is very unlikely to matter. If you're thinking about a concentrated piperine supplement, just run it by your pharmacist or doctor first.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Vituz combines hydrocodone, a narcotic cough medicine, with chlorpheniramine, an antihistamine. Hydrocodone is partly handled by the liver enzyme CYP3A4, and lab studies suggest piperine (the compound in white pepper) can slow that enzyme down a bit. In theory, slowing it could let a little more hydrocodone build up, which might mean stronger sedation, more drowsiness, dizziness, constipation, or slowed breathing.

Here's the reassuring part: this is only a theoretical concern from test-tube data, not real patient reports. The sprinkle of white pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are a different story. Since this drug can affect breathing, it's worth a quick chat with your pharmacist or doctor before adding any pepper-based supplement.
