Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for your inhaler. Ciclesonide is broken down by a liver enzyme called CYP3A4. The piperine in white pepper has shown, in lab dishes only, that it can slow that same enzyme down. In theory, that could let a bit more active ciclesonide hang around than usual.

The reassuring part: ciclesonide is an inhaled steroid, so very little reaches your bloodstream to begin with, and this concern is purely theoretical, not seen in real people. Normal cooking amounts of white pepper almost certainly won't matter. Even with concentrated piperine supplements, big problems are unlikely, though in heavy use you could in theory see more steroid-type effects like oral thrush, hoarseness, or throat irritation. If you're considering pepper supplements, just run it by your pharmacist first.

Alvesco (ciclesonide oral inhalation) is used for maintenance treatment of asthma. It is a preventive medicine for adults and teens 12 years of age and older.
