Empagliflozin falls under ANTIDIABETES DRUGS — the group White Pepper interacts with.

Evidence from animal research suggests that piperine, a constituent of white pepper, reduces blood glucose levels. Theoretically, white pepper might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the gist with Jardiance (empagliflozin) and white pepper. The concern isn't about pepper changing how your body breaks the drug down. It's simpler than that: in animal studies, piperine (the spicy compound in pepper) seemed to nudge blood sugar a little lower on its own. Stack that on top of a medication that already lowers blood sugar, and in theory the two could add up.

I want to be honest about how shaky this evidence is. It comes from lab and animal work, not people, so I'd file it as theoretical. Sprinkling white pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are the bigger question mark. If you're starting one, just keep an eye on your blood sugar and check in with your pharmacist or doctor first.

About Empagliflozin

What Is Empagliflozin Used For?

Brand name: Jardiance

Jardiance (empagliflozin) tablets are approved for several important uses in adults. It reduces the risk of cardiovascular death and hospitalization in adults with heart failure. It also slows the progression of chronic kidney disease by reducing the risk of serious kidney decline, end-stage kidney disease, cardiovascular death, and hospitalization. In adults with type 2 diabetes and established cardiovascular disease, Jardiance reduces the risk of cardiovascular death.

Jardiance is also used — alongside diet and exercise — to improve blood sugar control in type 2 diabetes in adults and children aged 10 and older. It is not recommended for blood sugar control in type 1 diabetes (it may raise the risk of a dangerous condition called diabetic ketoacidosis) or in type 2 diabetes patients with severely reduced kidney function.
