Estradiol, Norgestimate falls under P-GLYCOPROTEIN SUBSTRATES — the group White Pepper interacts with.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist with Ortho-Prefest (estradiol plus norgestimate) and white pepper. The piperine in pepper can, at least in a test tube, block a transporter called P-glycoprotein, which helps move certain drugs out of your body. If that pump slows down, in theory a bit more of your hormone medication could stay in your system. That might nudge up the usual hormone-related effects, like breast tenderness, nausea, bloating, headaches, or mood changes.

I want to be honest though: this is theoretical, based on lab research, not real patients. The sprinkle of white pepper you use in cooking is very unlikely to matter. If you're thinking about concentrated piperine or pepper-based supplements, just run it past your pharmacist or doctor first.
