Fluticasone, Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your Advair (fluticasone plus salmeterol): both of these inhaled medicines are cleared by a liver enzyme called CYP3A4. A compound in white pepper called piperine may slow that enzyme down a little. In theory, that could let more of these drugs hang around in your body, nudging up both their effects and their side effects.
With higher fluticasone levels you might see things like a sore mouth, hoarse voice, or thrush, and with more salmeterol you could notice jitteriness, a faster heartbeat, or shaky hands. That said, this is theoretical, based only on lab studies, not real patients. Pepper sprinkled on your food almost certainly won't matter. If you're thinking about concentrated piperine supplements, just run it by your pharmacist or doctor first.
