Fluticasone, Salmeterol falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Advair contains two medicines, but the one that matters here is salmeterol, a long-acting bronchodilator. Salmeterol is a stimulant-type drug that can nudge your heart rate up and, in some people, slightly stretch out the heart's electrical cycle (what we call the QT interval).

Ephedra is also a strong stimulant, and on its own it has been shown to cause heart rhythm changes, including a longer QT, even at normal doses. Stacking ephedra on top of salmeterol could add these effects together, raising your chance of a fast heartbeat, palpitations, jitteriness, or a more serious irregular rhythm. The evidence here is mostly theoretical, but the risk is taken seriously. Honestly, ephedra is banned in U.S. supplements for safety reasons. Please talk with your pharmacist or doctor before combining anything like this.

Advair is a combo inhaler. The salmeterol part is a long-acting beta-agonist, meaning it gently stimulates the same 'fight or flight' receptors that ephedra's ephedrine ramps up hard. So if you stack ephedra on top of your inhaler, you're essentially doubling down on that stimulation. In theory that could push your heart to beat faster and harder, trigger palpitations or a jittery, shaky feeling, and nudge your blood pressure up.

To be clear, this concern is based on pharmacology and not strong human studies, so it's more of a 'why risk it' situation. The bigger point is that ephedra itself is banned in U.S. supplements because of serious heart and stroke risks. Please don't combine these, and chat with your pharmacist about safe, approved options.
