Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for fosaprepitant (Emend Injection), the anti-nausea drug you usually get during chemotherapy. It's processed by a liver enzyme called CYP3A4. Lab studies hint that piperine, the spicy compound in white pepper, may slow that enzyme down a bit. In theory, that could let fosaprepitant linger a little longer and bump up its effects, like fatigue, hiccups, headache, or stomach upset.
But honestly, I wouldn't lose sleep over this one. The evidence is purely theoretical, based on test-tube work, not real people. The amount of white pepper you sprinkle on food is very unlikely to matter. If you're considering concentrated piperine supplements, just mention it to your oncology pharmacist or doctor first so they can weigh in.
