Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something worth knowing about this combination. Your cough syrup contains hydrocodone, which is actually a prodrug. That means your body uses an enzyme called CYP2D6 to turn it into hydromorphone, the form that does most of the pain and cough relief. White pepper's piperine may mildly block that same enzyme, at least in lab studies. In theory, that could mean your body activates a little less of the hydrocodone, so you might get less cough relief than expected.
Honestly though, this is based only on test-tube research, and normal cooking amounts of pepper are very unlikely to matter. Concentrated piperine supplements are the bigger question mark. If you take those regularly, just mention it to your pharmacist or doctor before relying on this medicine.

Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with this combo: piperine, the spicy compound in white pepper, may block an enzyme called CYP3A4 that helps your body process the hydrocodone in P-V-Tussin or Rezira. If that enzyme slows down, a little more hydrocodone could linger in your system. In theory that might nudge up its effects, including drowsiness, dizziness, constipation, or slowed breathing. The pseudoephedrine side is less of a concern here.
Now, the honest part: this is based on lab dish studies, not real people, so it's mostly theoretical. The pinch of white pepper on your dinner almost certainly won't matter. Concentrated piperine supplements are where I'd pause. If you use those, just run it by your pharmacist or doctor first.
