Valbenazine Tosylate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist for Ingrezza (valbenazine). Your body partly relies on an enzyme called CYP2D6 to clear this medicine. Lab studies hint that piperine, the spicy compound in white pepper, can slow that enzyme down. If that happened, valbenazine could build up a bit higher than expected, which might mean more of its known side effects, things like sleepiness, fatigue, balance trouble, or a feeling of being off.

The reassuring part: this is only a theoretical concern from test-tube work, not real patient cases, and ordinary culinary amounts of white pepper are very unlikely to matter. Concentrated piperine supplements are where I'd pause. Before starting one, just run it by your pharmacist or doctor so nothing changes without a check.

Valbenazine Tosylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the simple version: your body uses a liver enzyme called CYP3A4 to help break down valbenazine (Ingrezza). The piperine in white pepper has shown, in lab dish studies, that it can slow that enzyme down a bit. If the enzyme works slower, a little more valbenazine could hang around in your system, which in theory could nudge up its effects and side effects, things like drowsiness, dizziness, or feeling off balance.

The good news is that this is only a theoretical concern based on lab data, not real-world reports, and the pinch of white pepper you sprinkle on food is very unlikely to matter. Concentrated piperine supplements are a different story. If you're considering those, just check in with your pharmacist or doctor first.

What Is Valbenazine Used For?

Brand name: Ingrezza

Valbenazine — sold as Ingrezza and INGREZZA Sprinkle — is approved for two conditions in adults. The first is tardive dyskinesia, a movement disorder that causes uncontrolled, repetitive facial or body movements, often brought on by long-term use of certain psychiatric medications. The second is chorea associated with Huntington's disease, which refers to the involuntary, dance-like jerking movements that are a hallmark of that condition.

Both Ingrezza and INGREZZA Sprinkle are approved for both of these uses.
