Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear out vardenafil (Levitra). Piperine, the spicy compound in white pepper, has shown in test-tube studies that it can slow that same enzyme down. If the enzyme works more slowly, vardenafil could linger longer and reach higher levels than expected, which may bump up its usual side effects like headache, facial flushing, a stuffy nose, dizziness, or back pain.
The good news is that this is only a theoretical concern based on lab work, not real patient reports. Sprinkling white pepper on your food almost certainly won't cause trouble. Concentrated piperine supplements are the bigger question mark, so check with your pharmacist or doctor before combining those.
