Exemestane (Aromasin) is an aromatase inhibitor for hormone-sensitive breast cancer, and because its job is keeping estrogen suppressed, anything that shifts its blood level puts the treatment itself at risk. This list deserves real care. Three of the 141 interactions on file are rated major: grapefruit and St. John's wort can push the drug's level up or down through the liver enzyme (CYP3A4) that clears it, and activated charcoal can soak up the dose before it reaches your bloodstream.

Most of the remaining 98 moderate and 40 minor entries work that same enzyme in quieter ways, nudging exemestane's level in one direction or the other. A smaller group of bulky or binding products can blunt absorption of anything swallowed, so space those well apart from your dose. And five ingredients suppress aromatase on their own, which could stack with exemestane and deepen the low-estrogen side effects it already causes.
